Dear Parent/Guardian:
Medical
examinations for Danceline/Kickline/Pom/Funk for the 2007-2008 school
year will be given at Commack High School School. The
examination will be given by our SCHOOL PHYSICIAN. The
physical examination must be completed and passed before a student may
participate in try-outs for any of our teams. It will be the
responsibility of your daughter to get to the examination at the prescribed
time.
If your child wears contact lenses, braces, has a retainer or a dental cap,
a contact lens-orthodontic appliance permission note signed by a parent must
be submitted at the time of the examination by the school doctor.
Please complete the attached forms and have your daughter return them when
she reports for the medical examination. Additional sports physical packets
are available in the Nurse's Office.

#2 - Medical History Form which includes
the contact lens orthodontic appliance permission slip - Signed
Any recommendation that your family doctor may have regarding your
youngster's health and ability to participate in sports should be included
with the above list of forms returned to the Nurse's Office (contact
lens, capped teeth, etc.). The school physician is the final authority
regarding a student's fitness for participation in Danceline/Kickline/Pom.
Individual
parents/ youngsters may elect to have their physicals completed by your
family doctor. This IS PERMITTED but the physical must be documented on
school district forms ONLY. These forms must be filled out completely.
Completed documents MUST BE viewed and APPROVED by the schools physician
prior to receiving clearance for any tryout. It will be the responsibility
of the parent/youngster to ensure these voluntary physicals and documents
are completed to the standards of the school physician and district.
Completed forms should be submitted for review during the scheduled physical
dates/ times. Please alert the supervisors as you arrive.
Please refer to the reverse side of this letter for the dates and times your
youngster is scheduled for a physical. Times listed as "open" are for boys
or girls in the grades listed. Grades listed refer to your youngster's
level during the 2007-2008 school year.
REMINDER:
STUDENTS WILL
BE UNABLE TO OBTAIN SCHOOL PHYSICALS FOR ATHLETICS UNLESS THEY HAVE
COMPLETED ALL REQUIRED IMMUNIZATIONS ACCORDING TO STATE LAW. IF A STUDENT
IS UNABLE TO ATTEND ONE OF THE SUMMER PHYSICAL DATES, SHE WILL BE UNABLE TO
OBTAIN A PHYSICAL EXAM UNTIL THE WEEK OF SEPTEMBER 10, 2007.
