Commack High School Senior Survey Instructions
All seniors must complete their Senior Survey before they can receive their cap and gown. To do so:
1. Click “Naviance Family Connection Website” 2. If you have forgotten your account password, there is a link on the Family Connection main page that you can click to reset your account. That link is in the box labeled “Sign In for Returning Users.” If you have forgotten the e-mail address you used to register, or need further assistance, please contact your counselor. 3. Once you have accessed your account Click “Graduation Survey” 4. Step 1 · Select your plan for after high school from the drop-down box 5. Click “Continue” 6. Step 2 · Update or correct information about the colleges to which you have applied · List any other colleges to which you have applied in the space provided · Update the results of each application * Select final result from drop-down box * Check “WL” if you were waitlisted * Check “Defer” if you were deferred. 7. Step 3 · Important: If you indicated that you plan to attend college in Step 1 select the college you plan to attend from the drop-down box so that the Counseling Center knows where to send your final transcript. If you are not sure about your plans or if your plans change, return to the Survey and update this information. · Indicate your intended major in the space provided · List other scholarships or grants you have been offered even if you do not intend to go to the college that gave you the award. Please enter all dollar amounts without any commas, decimal points or dollar signs (Enter "$3,000.00" as "3000").” · If the award offered is renewable for all four years please enter the 4-year total value (ex. If the award is 5000 per year enter 20000) 8. Click “Continue”. 9. Remember to return to the Survey if you receive additional application results and scholarship information, decide which college you will attend, or change your mind about which college you will attend.
