Dear Parent/Guardian:
In 1993 the State Legislature approved legislation that mandates a breakfast program effective September, 1996 for all elementary schools that participate in the National School Lunch Program. The district need not initiate a breakfast program, however, if only a small number of parents request such a program.
A breakfast program includes milk, fruit or fruit juice, two servings of bread or meat/protein, or one serving of bread and one serving of a protein product such as cheese, eggs, peanut butter, etc. Breakfast, if offered, must be served before the first academic period or class activity begins each morning.
The price of a breakfast is estimated to be between $1.25 and $1.35. Students who are eligible for free or reduced lunch would receive free or reduced breakfast as well.
The implementation of a breakfast program requires a great deal of planning. It will require hiring of additional aides, and reorientation of the academic school day. Further, it may necessitate rebidding of our current food service contract. This could in turn result in a change in the price of school lunches. Accordingly, your response to the attached survey, positive or negative, is critical.
Please take a few minutes to complete the attached survey. Return it with your child as soon as possible. Your opinion is important and will help us serve both your child and the community in the best possible manner.
Very truly
yours, Laura A. Newman

COMMACK PUBLIC SCHOOLS
BREAKFAST
SURVEY The Commack Union Free School District is considering a school breakfast program for our students, grades K-8. This is in conjunction with recent directives from the State Education Department. Breakfast would consist of fruit or juice, cold cereal, muffins or bagels, a protein such as eggs, cheese or peanut butter, and milk. To determine if such a program would be utilized by our students, we ask parents to complete the following survey. The results of the survey will be used to evaluate the need for such a program. Please respond. Your input is vital. Transportation and school starting time will not be affected by a breakfast program. 1. Assuming a cost in the $1.25 - $1.35 range, would you expect your child to regularly buy breakfast in school? (Students who are eligible for free or reduced lunch would receive breakfast on a free or reduced basis.): YES NO 2. If you checked yes to the above question, how frequently do you think your child might buy breakfast? Almost Every Day Two or Three Days/Week One Day/Week 3. What grade and school is your child currently in?
Please complete one
survey for each child you have in grades K-8 and return it to your
child's teacher as soon as possible,
