The information on this sheet will be used in the 2004-05 Yearbook.  Please be honest and forthright.  The staff reserves the right to edit inappropriate information or language.  Please email your information NO LATER THAN October 1^st.   (If you do not have access to an email account you may hand in your information to the yearbook office.)

Please review the directions below:

1.    You may use any email account system i.e. AOL, MSN, Earth link, etc.

2.    Send your senior information to the email account chsyearbook@commack.k12.ny.us

3.    In the “subject box” please write “senior information” & “last name and first name”

 In the “body” of the email message please provide the following information: 

1. NAME (Full Name)

2. NICKNAME (Only Two)

3. ACTIVITIES (Only School Related; Abbreviate Where Possible) 

4. GOALS (immediate goals only) – Business, Armed Forces, College.  You can list one college major.   No long range plans—Astronaut, relief pitcher for the Yankees, etc.  AVOID GENERALIZATIONS – “UNDYING HAPPINESS” or “TO ACQUIRE A BILLION DOLLARS.” 

5. QUOTE (Avoid questionable language, references to drugs or alcohol.  This is quote not a personable message.  No “Harry, I Love YOU.”  Quotes cannot be more than 15 words.)

A T T E N T I O N !

Starting in September, we will begin to collect BABY PICTURES.  Please put your name and homeroom odd day on the back and place them in an envelope.  Return them to the yearbook mail box in the main office.  We CAN’T guarantee the return of these pictures.
