Grab and Go Meals - You Must Complete the New Survey to Participate!

AS OF MONDAY MAY 18, 2020 YOU MUST COMPLETE THE NEW SURVEY TO COLLECT GRAB AND GO MEALS.

 

If you would like to receive or continue to receive Grab and Go meals, you must complete the new survey below.

 

Please read all instructions carefully as procedures have changed.

We are happy and proud that we are able to support our students and children in the Commack community. In maintaining compliance with NYS Child Nutrition Office, we are making a few changes to our Grab and Go Meals program. First and foremost, starting Monday May 18, 2020, YOU WILL NEED TO COMPLETE THE NEW SURVEY TO COLLECT GRAB AND GO MEALS. We request that you view the bus routes at the link below and fill out the survey to inform us of which bus route (OR Commack High School location) you will be using in order to collect your Grab and Go Meals. Everyone regardless of having previously signed up must complete the survey. Please fill the form out completely, as employees working the delivery buses and high school loop will have rosters to check off that you have collected your meals when they meet you to hand out the meals. If you are not on the roster at the location you selected, you will be asked to go to the district website and sign up.

We ask that you print out the meal count sheet below and attach it to a closed window in your vehicle when collecting your Grab and Go meals.  (These are also being handed out as flyers when collecting your grab and go meals this week). If you do not have access to a printer, please write the following information on a piece of paper to display in your window:

Number of Meals

Please be sure that this information posted in your window matches the survey you submitted.

How to Get Meals (Buses)

  • Students and/or families should go to your chosen bus route stop (Routes A through L).  Younger students should be accompanied by an adult.
  • Please account for approximately 3-5 minutes for each stop; bus drivers will wait a few minutes at each location to give time for families to come out.
  • Bus stops are listed in order in the links below and you can estimate the approximate time of delivery by counting the stops and multiplying by three.
  • Buses will depart the high school at 10 am with the first stop listed at the top of each individual bus list (Bus A, Bus B, Bus C, etc.) Again, please allow approximately 3-5 minutes for each stop (estimated). So if your stop is Bus A in your catchment zone, then you should be at the stop at 10 am. If your stop is the 10th spot, then give at least 30 minutes before heading to the stop.
  • Please remember to practice social/physical distancing at each bus stop with others who may be waiting for bus meal delivery.
  • If you are in a car, please remain in the car and bus staff will verify you have signed up and hand the meals to you through the passenger window or place in the trunk.

How to Get Meals (High School)

  • If you are collecting grab and go meals from the high school loop, meal handouts will take place between 10:00AM and 12:00PM daily.
  • Please remain in a single file line when driving through the bus loop.
  • Please follow the guidelines for the flyer being placed in a closed window. This will help to expedite the verification process of our high school pick up roster and also the meal distribution process.
